Terms of Use

These terms govern your use of the Forma mobile app (the “App”). By downloading or using the App you accept these terms. If you do not accept them, please do not use the App.

1. Scope of service

Forma is a wellness and motivation app offering AI-powered personal nutrition plans, workout programs, calorie/weight/water tracking and coaching chat. The free tier includes limited features; all features unlock with a Premium subscription.

2. Health disclaimer — please read

Forma is not a medical service. The plans, suggestions, calorie and macro figures in the App are estimates; they are not a substitute for medical advice, diagnosis or treatment. Consult your doctor before starting any diet or exercise program. If you are pregnant, have a chronic illness, a history of eating disorders, or take medication regularly, use the App only with a health professional's approval. By using the App you accept that you apply the plans and suggestions at your own responsibility and that Forma is not liable for any health outcomes.

3. Account

A valid email is required to register; you may also sign in with your Apple or Google account.
You are responsible for the confidentiality of your account and for activity under it.
The App is for users aged 13 and over.
You can permanently delete your account any time in the App (Profile → Help & support → Delete my account).

4. Premium subscription

Premium is offered as a monthly (₺149.99) or yearly (₺899.99) subscription. Current prices are shown on the purchase screen and may vary by region.
Payment is charged to your App Store account upon confirmation of purchase.
Subscriptions auto-renew unless cancelled at least 24 hours before the end of the period; the renewal fee is charged within the 24 hours before the period ends.
Manage or cancel your subscription in Settings → Apple ID → Subscriptions on your iPhone. On cancellation, Premium stays active until the end of the current period.
Refunds are subject to Apple's policies and can be requested at reportaproblem.apple.com.

5. Acceptable use

You may not use the App for any unlawful purpose.
You may not interfere with the App's operation, reverse engineer it, or attempt to bypass security.
You may not use any feature, including the AI coach, to produce harmful, misleading or abusive content.

6. AI content

Plans, analyses and chat responses are generated by AI and may contain errors. Accuracy, completeness or fitness for a particular purpose is not guaranteed. Calorie values from photo analysis are approximate estimates.

7. Intellectual property

The App, its design, brand and content belong to Forma. You are granted only a personal, non-transferable, non-exclusive right to use it; the App may not be copied, sold or distributed.

8. Limitation of liability

The App is provided “as is”. It is not guaranteed to be uninterrupted or error-free. To the maximum extent permitted by law, we are not liable for indirect, incidental or consequential damages arising from use or inability to use the App. In any case our total liability is limited to the amount you paid for the App in the last 12 months.

9. Changes and termination

We may update these terms and the App's features from time to time; significant changes are announced in the App. We reserve the right to suspend or terminate accounts that violate these terms.

10. Governing law

These terms are governed by the laws of the Republic of Türkiye; the courts of Istanbul have jurisdiction over disputes.
